We help those in need with hygiene, used clothing, food, and other needs if community donations have it available. We have a Thanksgiving meal, Christmas meal/gifts Outreach annually for low-income families and grandparents, and veterans. We also do an Easter meal/Basket outreach for families, grandparents, and veterans. Our Grandparents' programs offer one Christmas gift and/or Easter basket during event application.

Offers developmental evaluations to parents who have concerns about a child's development. Evaluations are completed by trained professionals and consider a child's speech, social behaviors, attention span, play, self-care, vision, hearing and overall behavior. Comprehensive developmental evaluations are available for children ages birth-6. For children under age 3, there are no out-of-pocket expense to determine eligibility for the early support program.

Provides wildlife/environmental presentations to schools, civic organizations, scout groups, etc.  Offers all-school assembly presentations; in-classroom presentations; and in-field programs led by rangers.

Provides quality, timely, comprehensive medical care in the patient’s home. By providing physical, emotional, and spiritual support, our trained staff and volunteers assist the patient and family as they work towards medical rehabilitation or a peaceful and dignified death. We work to promote optimum patient comfort through pain and symptom management, palliative care, stress reduction, and the general well-being of the patient.

Provides case management services to Snohomish County seniors and disabled persons 18 and older who require multiple services and are unable to obtain them without the assistance of a case manager.  Services include in-home assessment, development of an individualized plan of care, and follow-up to assure that clients receive the appropriate services for their needs.

Complete case management services include support with healthcare/medication access, transportation, housing, legal, substance use, and adjustment or mental health needs related to HIV/AIDS. Specialist on staff works to connect patients with health insurance, DSHS benefits, Social Security Administration benefits, and state HIV/AIDS programs for clients.   Outreach services are provided to the greater King County area.   Clinic social workers are masters level clinicians who have experience working with multidiagnosed clients, including those with substance use, adjustment, and mental health needs.   People who meet selection guidelines are welcome to participate in research studies designed to improve diagnosis and treatment of HIV/AIDS.

Facilitates social work case management services and advocates on behalf of clients seeking mental health, substance use, and medical treatment. Provides comprehensive psychosocial assessments, develops and monitors care plans, and offers service coordination, sexual health and prevention counseling, as well as back-to-work counseling and referrals. Assists clients in accessing community services and public benefits, and helps coordinate care across outpatient and inpatient settings. Additionally, provides financial advocacy by helping patients access income and medical coverage benefits, supporting benefits management, networking with DSHS and Social Security, guiding back-to-work planning, and coordinating with hospital financial counselors and healthcare coverage services.

Supplements center based services: this program - informs, - educates, - supports and assists Service members (Active, Guard and Reserve), and their Families who are geographically or socially isolated from a military installation.

The primary goal of the CARES Team is to improve quality of life and reduce unnecessary use of the emergency health care system by addressing a client’s needs based on the social determinants of health. Many times, Fire Department personnel encounter an individual who has significant needs apart from their physical health needs. When they encounter these situations, Fire Department EMS providers make a referral to the CARES Team requesting a follow-up visit.

Provides connection to housing options for teens in crisis ages 12-17.

The New Parent Support Team (NPST) is a voluntary program for expectant/new Navy parents.  Home visitation is the core service, with a focus on the health needs of infants and toddlers, prenatal and postnatal care, parent education, family/social support, and resources within the community.  Services focus on supporting the parent as well as parent-child interaction and child development.

Program is designed to prevent intimate partner violence in the community. A Prevention Advocate works with groups like schools, faith institutions, sports teams, and other organizations to create and implement material related to helping youth develop healthy relationships.
