Determines eligibility for state assistance programs that help participants pay for attendant care and non-care related assistance services designed to help older adults and adults with disabilities remain independent.

Assistance may be provided in any of three settings:
- Participant's home
- Assisted living facility
- Adult family home
Community First Choice
- Provides attendant care and non-care related assistance services to older adults who would otherwise require nursing home placement
- Program participants can choose from whom they receive care services, including hiring family members as care providers
Community Options Program Entry System (COPES)
- Generally provided to clients who are also enrolled in Community First Choice
Medicaid Personal Care
- Provides personal care services for clients who do not require nursing-home-level care
Program of All-Inclusive Care for the Elderly (PACE)
- Offers all services through one provider.
- PACE can be funded either through Medicaid, Medicare or private-pay.
New Freedom
- Participants manage their own service plan and budget and take primary responsibility for obtaining services.
Medicaid Transformation Demonstration Medicaid Alternative Care (MAC)
- Supports unpaid caregivers of Medicaid recipients over the age of 55 Targeted Supports for Older Adults (TSOA)

Provides treatment via an app, web cam or phone for a variety of minor illnesses. Able to prescribe medications, excluding narcotics and anxiety medication. Payment via subscription model for app. Web or phone charged as regular doctor visits.
Provides treatment via web camera, phone or app for a variety of minor illnesses such as asthma, back pain, pink eye, sore throat, seasonal allergies and flu. Virtual visits should not replace a relationship with a primary care provider.
Prescribes medications including antibiotics when indicated, allergy medications, anti-nausea medications, and various skin creams or ointments. UW Medicine Virtual Clinic providers will not prescribe or refill medications for the treatment of a psychiatric condition, and they will not prescribe controlled substances, including medications such as:
- Narcotic medications (i.e. Oxycodone, Vicodin, Percocet, Codeine)
- Anti-anxiety medications (i.e. Xanax, Valium, Ativan, Klonopin)
Visits typically last about 20 minutes.

Operates a national hotline where people about to use substances and potentially overdose can call. If the caller stops responding, the operator will reach out to EMS to go out to a caller's location to help.

Callers will be asked for their first name, phone number, locations, county, and type of substance being used. This information is kept private unless the individual stops responding, in which the call handlers will reach out to EMS to go to the callers location to help.

Cosechando Esperanza is a food and clothing bank that was establish in 2020. It is a culturally appropriate Hispanic/Latino food bank that operates weekly. Also has a clothing and diaper bank that operates every 2 weeks.
Esperanza offers a wide range of social and human services through Resource Navigators and Social Work Practice. A social service and wellness program that advances food security, family healing, mental health, and LGBTQ supports for adults, youth, and families.
