Manages state assistance programs that help participants pay for attendant care and non-care related assistance services designed to help older adults and people with disabilities remain independent.   Assistance may be provided in any of three settings:   -  Participant's home   -  Assisted living facility -  Adult family home   Community First Choice -  Provides attendant care and non-care related assistance services to older adults who would otherwise require nursing home placement    -  Program participants can choose from whom they receive care services, including hiring family members as care providers     Community Options Program Entry System (COPES) -  Generally provided to clients who are also enrolled in Community First Choice   Medicaid Personal Care -  Provides personal care services for clients who do not require nursing-home-level care   Program of All-Inclusive Care for the Elderly (PACE) -  Offers all services through one provider.   -  PACE can be funded either through Medicaid, Medicare or private-pay.     New Freedom -  Participants manage their own service plan and budget and take primary responsibility for obtaining services.   Medicaid Transformation Demonstration Medicaid Alternative Care (MAC) - Supports unpaid caregivers of Medicaid recipients over the age of 55 Targeted Supports for Older Adults (TSOA) - Supports older adults over the age of 55 who do not receive Medicaid  Participants enrolled in one of the in-home care programs may also qualify for other forms of support including assistive technology and personal emergency response systems.

Offers cost-effective in-home care services as an alternative to facility-based nursing, enabling them to maintain their independence in their own homes. In-home care services provide consumers on-going assistance with activities of daily living, which may include services like: bathing, dressing, grooming, meal preparation, housekeeping, errands/transportation, and medication reminders. These supportive services can be arranged for any frequency of time, from one to twenty-four hours of care daily, or on a respite or temporary basis. Addus works with clients, their families and other relevant health care providers to customize care plans, designed to keep individuals safe, healthy and independent at home. These services are funded through a variety of sources, including: Medicaid waiver programs, long-term care insurance, veterans benefits, private pay, and other location-specific social service programs.

Provides case management services by social workers with nursing staff assistance which includes functional eligibility assessment, personal care, housekeeping, errand assistance, home delivered meals, home modifications, and personal emergency response system (fall button).

Provides licensed non-medical personal care needed to remain independent and live safely in own home. Available 24 hours daily. Provides assistance with light housekeeping; laundry, ironing, and changing bed linens; shopping and errands; bathing, grooming & incontinence care; standing and walking; medication reminders; diet maintenance (meal planning, preparation & cleanup); transferring & positioning; use of hoyer or other lift devices; companionship and supervision; dressing; eating; recreational activities, walks & outings; arranging for & transportation to local appointments to doctor visits, pharmacies, hairdressers/barbers, or for visits with friends. Also provides nurse delegated care, family care, respite for family caregivers so they can have time to take care of other responsibilities or take time for themselves, supportive care for developmentally disabled individuals, and Alzheimer and related dementia care.
