The Supportive Living Program has been providing critical services to individuals who are homeless with serious or chronic mental illness to help them live more stable, productive lives through a combination of behavioral health services and housing supports. Step-by-step in locating housing, completing applications, interviews, compliance paperwork, and lease signing. Assist individuals in finding, securing, and maintaining housing by overcoming barriers with past evictions, criminal and credit issues.

Provides several services to Developmental Disabilities Administration (DDA) clients, including Staff and Family Consultation and Respite care for DDA clients. Other services include: Specialized Habilitation Helps individuals create healthy relationships, adapt to challenges, and improve overall quality-of-life. Services offer resources for community integration including: - Self-empowerment. - Safety awareness and self-advocacy. - Interpersonal skills and effective communication. - Coping strategies regarding typical life challenges. - Learning how to manage daily tasks to gain adaptive skills. Community Engagement Services are designed to develop creative, flexible and supportive community resources and relationships to connect individuals to resources in their community, and supports them to participate, engage, and integrate into the community. A community guide or community engagement provider will: - Meet with you to discuss goals stated in the Person-Centered service plan and help you develop specific goals related to engaging with the community. - Develop strategies with you to connect with community resources, based on your interests and needs. - Help you develop healthy relationships with local community members. - Support you with participating community activities within you budget. Peer mentoring Offers a form of mentorship that takes place between a person who is living through the experiences of having a developmental disability or a family member of a person who has a developmental disability (peer mentor) and a person who is new to the experience (peer mentee). These services help individuals or family members through: - Mentoring. - Sharing personal experiences to provide support. - Sharing information about community resources. - Exploring other supports. Specialized Goods and Services Provides participants and their families with specialized supports to achieve stabilization in their home and community and may include: - Vehicle modifications or adaptations required to fit the needs of the participant to engage with their community. - Therapeutic equipment and supplies necessary to implement a behavioral support plan or other therapeutic plan including sensory integration or communication therapy plan. ​ - Nonrestrictive clothing adaptive to the participant's individual needs and related to his/her disability. - Purchasing or leasing, and customizing or adapting assistive technology to meet the functional needs of a child with a disability. ​ ​

Provides twenty-four-hour support services and case management to adults with developmental disabilities. Provides support and education in areas such as: -  Meal-planning, nutrition, cooking -  Shopping, budgeting, bill-paying, banking   -  Cleaning, laundry -  Exercise -  Social skills -  Using public transportation   Provides financial and case management to make sure our clients: -  Have desirable jobs -  Social security income -  Subsidized housing -  Utility assistance -  Excellent medical care -  Mental health services when needed   Supports and encourages clients to enjoy recreation, enrichment, and leisure activities such as:   -  Joining sports programs -  Attending classes and cultural events -  Volunteering their assistance to worthy causes

Federal program for navigating the Long Term Care (LTC) services and processes so that the individual may receive any desired help with meeting their personal care needs including getting dressed, driving to appointments, or making meals. Offers Pathfinder services to help find providers in the area as well as different types of services depending on special circumstances, needs, or demographics.

Provide training, assisted devices, and support to enable people experiencing vision loss to maintain or regain their independence. Services are individualized based on each person’s needs and goals.

Offers a variety of residential programs designed to allow individuals with disabilities to live in a community setting.   Participants live semi-independently in their own homes – houses or apartments – and have the opportunity to select their own menus, do or help out with their own cooking, and make decorative choices about their homes. Also helps assist with medications, coordinate and provide transportation for health care, provide support for finances, facilitate relationships with family and friends, and support involvement with social and recreational activities.

The program can help with deposits, utilities, and rent. The program can also help with move in expenses, such as purchasing a bed, household supplies, and other essentials. Staff also conducts a pre-move in inspection and an annual inspection to ensure that the unit is a safe and functional unit for habitation.

Provides one-on-one support so that adults with intellectual or developmental disabilities can live as independently as possible in the community. Based on assessment, may assist clients with tasks such as:   -  Home management -  Personal care and financial management -  Negotiating various services such as DSHS, vocational, medical, etc. -  Developing and maintaining relationships -  Exploring personal growth -  Obtaining and managing resources and services

Provides in-home and community support for developmentally disabled people living in their own home or apartment.

We provide skilled, compassionate, supportive living services for people with developmental disabilities, persistent mental illness, and brain injuries. JFS provides case management, life skills training, and non-medical in-home care (like assistance with housecleaning, meal preparation, shopping and transportation).

Provide training, assisted devices, and support to enable people experiencing vision loss to maintain or regain their independence. Services are individualized based on each person’s needs and goals.

Identifies and assists individuals residing in nursing homes and institutions to transition into community-based or more independent living situation by coordinating and locating available resources.

Provides training and support services for people with developmental disabilities in residential settings, whether in their own home or apartment. The types of training and levels of support are specifically tailored to the needs of each individual program participant. Services offered include: meal preparation, bathing and dressing, shopping, financial resource management, transportation, household assistance, personal hygiene, body care, eating assistance, toileting, self-medication assistance, and transfer positioning.

Provides permanent supported housing for homeless individuals, veterans, mental health diagnosis, and families. Also provides housing for individuals and families with low incomes.

Contracts with agencies to provide services and supports for older adults with developmental disabilities who are retired, or choose not to continue employment services after participating for at least 9 months.   Services help individuals   -  Provide the support necessary to build and strengthen relationships between family members and members of the local community who are not paid to be with the individual -  Find places where an individual’s interest, culture, talent, and gifts can be contributed and shared with others with similar interests -  Provide opportunities for people to do things they enjoy as well as new and interesting things   -  Support participation in clubs, associations, and organizations as members and in decision-making capabilities

Provides in home and community support for people who have a developmental disability.   Focuses on people who have a physical disability.

Provides support for people who have a developmental disability in achieving greater independence and fulfillment on their pathway to employment. - Coordination of DDA employment and community inclusion services ages 21+ - Transition Services for young adults with developmental disabilities who have turned 21 years old and have graduated from High School. Adult Services to assist adults with developmental disabilities to find and maintain community integrated employment and live full participating and contributing lives within their community. - Community Education and Information Services are designed to increase public awareness and build community acceptance for including people with developmental disabilities.

DAC NEW is an organization run by people with disabilities for people with disabilities. DAC NEW provides peer counseling, institutional transition, skills training, support, and mentoring for individuals with disabilities.
