- Chronic Disease Self-Management Program: Supports people with different or multiple chronic health problems (e.g., heart disease, hypertension, stroke, and respiratory disease). The program is led by trained and certified peers with first-hand experience. Discusses techniques on how to deal with frustration, isolation, appropriate exercise, fatigue, pain, medication management, emotion management, and much more.
- Chronic Pain Self-Management Program (CPSMP): Developed for people who have a primary/secondary diagnosis of chronic pain; which is pain that lasts longer than 3 to 6 months, or beyond the normal healing time of an injury. Examples of chronic pain conditions are chronic musculo-skeletal pain, fibromyalgia, whiplash injuries, chronic regional pain syndromes, chronic pelvic pain, post-surgical pain that lasts beyond 6 months, neuropathic pain, or neuralgias, post stroke pain, persistent headache, Crohn’s disease, irritable bowel syndrome, diabetic neuropathy, or those who experience severe muscular pain due to conditions such as multiple sclerosis.
- Diabetes Education and Support: Discusses techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and all of the emotional problems associated with this disease. Material has been reviewed by physicians, diabetes educators, dietitians, and other health professionals who are experts on the subject.

Prevent T2 is the YMCA’s signature and supportive, 12-month small group program where participants work together to learn about healthier eating and physical activity in order to reduce their risk for developing diabetes.
Offers a one-year, community-based, lifestyle intervention program for overweight adults at high risk of developing type 2 diabetes.
Designed to enable adults to adopt and maintain lifestyle changes that will improve their overall health and reduce their risk of developing type 2 diabetes.
Through the program, participants will receive support and encouragement from both a trained Lifestyle Coach and fellow participants as they develop a plan for improving and maintaining their overall well-being.
The YMCA's Diabetes Prevention Program is nationally supported by the Centers for Disease Control and Prevention (CDC) and other national organizations committed to preventing chronic disease.
