Para-Transit is a wheelchair-accessable transportation service for individuals whose disability prevents them from using the regular fixed route buses to medical appointments.
Provides case management services to all adult clients, regardless of age, who are receiving community-based services in their home, funded through Title XIX Medicaid and state-funded chore personal care. Activities include conducting assessments and re assessments, authorizing and reauthorizing services, assisting clients with the Medicaid eligibility review process, locating and hiring providers, arranging and preparing for fair hearings, as well as regular, ongoing case management activities.
Provide comprehensive dental care for adults and children starting at 6 months, including exams, cleanings, x-rays, and fillings.
