The program provides care coordination with medical and behavioral health as well as long term services and support through a person-centered action plan at no cost to the participant. Services include provider management, appointment assistance and connecting with community resources.

Enhance Fitness Program;  A group exercise program for seniors that offers low-cost, one hour supervised classes for on-going sessions of three classes per week.  Components of the class include strength training with wrist and ankle weights and aerobics, balance and stretching.  EnhanceWellness Program:  A comprehensive participant centered health behavior change program, which includes assessment of health status, personal guidance to improve health, and ongoing support to choose a healthier lifestyle.  A Registered Nurse and Social Worker work with participants on issues they have chosen to address.  Trained volunteers, called health mentors, assist participants, as requested, in achieving their desired health goals.

Improves the health and functioning of older adults through a low-cost, comprehensive system of health promotion and physical fitness programs.   Enhance Fitness Low-cost, evidenced-based group exercise program for older adults.  The program is designed to improve older adults physical conditioning level and decrease risk of falling.  Classes are held three times a week. Each hour-long class includes strength training, cardiovascular conditioning and balance and stretching exercises.  Lifetime Fitness Program is a program of Enhance Wellness Project of Sound Generations.   Balance Exercises Practice positions to build strength and balance. Helps participants increase balance and get started exercising. All abilities are welcome.  Yoga Provides several yoga class options - chair yoga, gentle yoga and laughter yoga - that help build strength and flexibility

A women-only facility that provides services such as fitness classes, massage, Reiki, Reflexology and Bowen therapies.

A Care Coordinator will spend time understanding participants’ needs and help participants and family connect to available resources such as housing, health insurance, childcare, food assistance programs, and other services. The services available differ by area.

Provides diabetes education, follow-ups, and support for individuals diagnosed with type 1 & 2 diabetes. Provide medication management, pre-diabetes education, lifestyle coaching and monthly support groups.

Offers a low-cost, comprehensive system of health promotion and disease management programs to improve the health and functioning of older adults.    Enhance Wellness Connects participants with a personal health and wellness coach to improve physical, emotional and social well-being. After completing a health assessment, participants develop a personalized Health Action Plan. The participant's physician is kept informed of progress throughout the program. Located at numerous sites in King County. Coaching lasts about six months. Enhance Fitness Group exercise program designed to improve older adults' physical conditioning level and decreases risk of falling. At The Everett Clinic you can take an American Diabetes Association-approved program of diabetes self-management education. Get a referral from your primary care provider and check with your insurance provider in advance to see if the education is covered. Classes are offered in Everett and Smokey Point.

Health Home services are available to adults or youth in Spokane County who are managing multiple chronic health conditions including, but not limited to, mental health and substance abuse disorders.

Washington State’s Supplemental Nutrition Assistance Program Education. It is a practical, hands on nutrition education program available to Washington Basic Food (food stamp) recipients and schools where 50% or more of their students receive free or reduced lunches. This nutrition education program reaches youth and adults in the Kitsap community with information to help participants practice healthy eating habits and choose physically active lifestyles.

Acupuncture, massage, chair exercises, dancercize, fun and fitness, aerobics, yoga, and zumba.

A group exercise program for seniors that offers low-cost, one hour supervised classes. This class lends itself to your fitness level, be it sitting, standing or a combination of both.

Coordinates workshops and health education sessions. Offers EnhanceFitness, yoga, strength training, and a seated fitness class.

Offers free blood pressure checks, weekly, massage therapy (Wednesdays except first Wednesday of the month) and wellness checks on the fourth Monday of the month (via partnership with POCAAN).

A low impact fitness class that focuses on cardiovascular exercise with strength, balance and stretching.

Services include: - Free Medical Clinic for uninsured low-income adults - Mobile dental clinic in partnership with PCHS (sliding scale cost paid to PCHS) - Diabetes class in Spanish - Monthly food/clothing distribution events - Resource navigation and referrals for housing, legal aid, education, employment, identification documents, financial management, charity care, and more - Limited temporary emergency rent assistance for existing clients - Carseats - Youth programming for middle- and high-school students - Annual holiday celebration - Community education and advocacy

Operates a Fitness Center and provides fitness programs and classes. Fitness Center provides cardiovascular and strength-training equipment specifically for older adults. A physical fitness trainer is available to work with participants.  Classes include  -  EnhanceFitness - an exercise program that includes balance, flexibility and aerobic training -  Matter of Balance - learn about fall hazards and exercise to improve strength and balance -  Qigong - Tai Chi -  Walking group -  Various yoga classes -  Strength class. Classes change quarterly.

Bridgeview Resource Center is a community building that houses multiple services through partnering agencies, including housing, employment, financial management, food, educational, health, aging and disability, and crisis services.
