A Care Coordinator will spend time understanding participants’ needs and help participants and family connect to available resources such as housing, health insurance, childcare, food assistance programs, and other services. The services available differ by area.

A group exercise program for seniors that offers low-cost, one hour supervised classes. This class lends itself to your fitness level, be it sitting, standing or a combination of both.

Coordinates a low-cost, comprehensive system of health promotion and disease management programs, including the Enhance Fitness Program, which offers group exercise for older adults. The program is designed to improve older adults' physical conditioning level and decrease risk of falling. Classes are held three times a week and each hour-long class includes strength training, cardiovascular conditioning, and balance and stretching exercises. Enhance Fitness Program is a program of Senior Wellness Project of Sound Generations and is offered primarily at senior centers throughout King County.

Coordinates group classes for those with Type 1, Type 2 or gestational diabetes. Includes information about carb counting, meal planning, understanding medications, and physical activity. Individual sessions are available for diabetes patients who are newly diagnosed or who have special needs.

Washington State’s Supplemental Nutrition Assistance Program Education. It is a practical, hands on nutrition education program available to Washington Basic Food (food stamp) recipients and schools where 50% or more of their students receive free or reduced lunches. This nutrition education program reaches youth and adults in the Kitsap community with information to help participants practice healthy eating habits and choose physically active lifestyles.

Provides prevention and education services and activities for patients at the facility and in the community. Offering education about diabetes education, cancer prevention screenings, hypertension (NOSH Program), and fitness and nutrition classes throughout the year.

Evidence-based classes (shown to improve strength and balance to prevent falls) include Enhance Fitness, Stay Active and Independent for Life (SAIL), and Tai Chi: Moving for Better Balance. Other classes include Yoga, Pilates, Tap. Line Dance, and Laughter Yoga.

The primary purpose is to promote health and prevent illness and injury in child care environments.  This plan includes assessments, consultation, training, communicable disease investigation and referral to community resources.
